Stroke.Fitness LLC operates as a “post-therapy functional training” or “physician-referred fitness” program. 

1. Purchase any of our services on Stroke.Fitness — either an intensive program or a membership — and retain the receipt for your records.

2. Print out the referral form below.

3. Allows a physician or specialist (e.g., neurologist, physiatrist, or general practitioner) to prescribe or recommend continued exercise by signing the document.

4. For tax-deductible wellness programs, present your receipt to the tax authority

5. For reimbursement through your insurance provider:  submit both the receipt and the form signed by the attending physician. 
6. Ask us if you need medically recognized terminology to support reimbursement  through your insurance provider.

7. The STROKE.FITNESS program is post-therapy, non-medical functional fitness. Patent pending.

8. Stroke.Fitness LLC cannot guarantee reimbursement or tax deduction eligibility. Approval is subject to the policies and discretion of the patient’s insurance provider or tax authority.
